YOUR FIRST VISIT

Please arrive at our office 15 minutes prior to your scheduled visit in order to complete the registration and medical forms.

Here is a list of items that you should bring with you for your first visit:
  • 1. Photo ID or driver’s license.
  • 2. Health insurance or Medicare Card.
  • 3. List of all current medications.
  • 4. List of all allergies.
  • 5. If available, any diagnostic studies that have been performed with regard to your current foot and ankle problems (i.e., blood tests, x-rays, bone scans, CT scans, MRI’s, etc.).
